Social Worker Needed in Jackson, Mississippi!

Adecco medical & Science is looking for an individual with Social Work or Social Services experience for a permanent position. This individual will assist the health plan in establishing a presence in the community: educate the members about managed care services provided via the health plan and how to access them: community outreach.

Duties include educating members about managed care services provided by the health plan, teach members how to access this care: order, deliver and teach members on use of a "Connection Plus" cell phone: scheduling home visits for noncompliance issues, informational visits, & assist with ED diversion programs. Conduct targeted phone calls for compliance, preventive health, health risk assessments, etc. Assist with finding & connecting members to community services/agencies. Ability to travel throughout your designated region.

Previous manged care or social services work experience preferred
Minimum 2 years experience in a social service setting
Valid, clean Drivers License and proof of insurance
Ability to travel
