Exciting Full Time job opportunity for a RN Case Manager in San Fernando, CA! $5,000 sign on bonus for eligible RN's!



Job Duties:


Provide case management for patients who experience catastrophic illness, who have complex chronic or high risk conditions.
The hours for this position are Monday-Friday from 8:30 am - 5:30 pm.

$5,000 sign on bonus!



Job Requirements:

RN graduate of an accredited school of nursing.
Minimum three years experience preferred in a managed care setting/utilization review/discharge planning/case management.
Strong clinical background, preferably in acute care.
Current CA RN licensure required.
American Heart Association BLS for Health Care Providers.
Bilingual – Spanish required.
