Specific Duties: Maintains medical record integrity and supports patient care and the billing process by filing, retrieving and releasing
confidential information in compliance with California Hospital Association guidelines, CHW guidelines, state and
federal laws. This position requires the full understanding and active participation in fulfilling the Mission of the
Organization. It is expected that the employee demonstrate behavior consistent with the Core Values.

Specific Duties:
1.1 Department receptionist and primary telephone contact.
1.2 Collect, maintain and protect Patient Health Information (PHI) in accordance with state and federal law and Dignity Health guidelines.
1.3 Act as department liaison between other hospital departments, CA Dept. of Social Services, copy services, attorneys and physicians as it relates
to department and position functions.
1.4 Process requests and subpoenas for patient information in compliance with state and federal law and Dignity Health guidelines, prioritizing the
work by urgency of need for patient care and due date.
1.5 Retrieve and distribute department mail.
1.6 Maintain necessary skills and knowledge of computer programs and applications needed to perform assigned tasks.
1.7 Ensure that all records to be scanned are received timely and follow up on missing records appropriately.
1.8 Scan all assigned records within two days of receipt, or other timeframe that has been approved by your manager.
1.9 Review records for quality and completeness, assigning and removing physician deficiencies as needed.
1.10 Other duties as assigned.

Must have qualifications/experience:
1. One-year experience in a clerical position required.
2. Experience in related position in healthcare

Shift Start/End Time: 8-4:30

Must have previous job-related computer
experience and be comfortable learning new
programs. Ability to effectively
communicate with department employees
and facility staff at all levels, written and
verbal. Ability to effectively communicate
with patients and physicians in a
professional manner. Capable of making
independent decisions within the scope of
assigned responsibility. Able to manage
multiple priorities and deadlines with
frequent interruptions. Good telephone
skills, including patience and
professionalism. Willingness to embrace
