A Coordinator of Clinical Services positionis now available in Bronx, NY through Adecco Medical & Science.

This is a temp contract role, with a possibility of an extension or temp to perm.

Specific Duties:
Coordinator of Social Services, (40) hours per week, Monday – Friday. The Coordinator of Social Services is on call twenty-four (24) hours per day/seven (7) days per week. A cellular phone will be provided to allow for communication and supervisory oversight as needed.
*Hours may vary based upon program and company needs. This position requires carrying a cellular phone for management coverage 24 hours per day / 7 days per week.

Position Objective:

The Coordinator of Clinical Services assumes responsibility for ensuring completion of QIDP and Social Service task required by agency operation, NYS OPWDD and necessary to ensure consumer rights and safety.
Ensure the provision of person-centered planning, consumer responsive services to the consumers and families served at CAHS, Inc. Coordinator of Clinical Services works to oversee and implement the Office of Mental Retardation and Developmental Disabilities (OPWDD) principles of three (3) IP’s and five (5) governing principles are in place for all individuals and families served.


Major Duties/Responsibilities:

As assigned by the AED or agency designee, responsibilities will include but not limited to the following:

*Workdays and hours can change depending upon agency needs, i.e. possible occasional Saturday(s) or evening hours.

Program and Administration:

Ensures through direct oversight and monitoring individual consumer services are person centered and responsive, that the program is rendering quality services to all individuals at maximum levels in all areas and in accordance with overall habiting services objectives. Ensures the delivery of such services incorporating person-centered planning, OPWDD, 3 IP’s and five governing principles.
Ensures timely application for all consumer benefit entitlements (Social Security, Medicaid Recertification, Medicare, Food Stamps, etc.) maintains consumer eligibility and advises supervisor of any problem which may affect entitlements.
Direct consumer services, i.e. case management, group facilitation, life tutors, planning and provision for recreation/respite, budgeting, etc.
Provide crisis intervention and prevention for consumer, if and as needed.
Conduct consumer/family intakes as necessary, per agency and OPWDD HCBS Waiver guidelines. Coordinates the process for Admissions/Referrals/Discharges in accordance with established procedure; participates in agency development and quality improvement activities.
*Flexible Hours (days/hours will vary based on agency and/or consumer needs)
Works with families, including making home visits to facilitate the effective natural supports and the consumer habilitation plans.
Preparation, coordination and participation in in-service training(s) and group counseling sessions for consumers, families and staff as needed.
Provide individual counseling for caseload.
Participate in ICF/IRA/Day Habilitation meetings and community meetings, as needed. Monitor and review the facility grids for the service plans or ISP meetings.
Monitor the administration of medications and service treatment plans for caseload.
Reviews and Co-Chairs committees such as SCC –Human Rights and participates in overseeing the Interdisciplinary Treatment Team meetings. Review the documentation and participates in developing and implementing corrective actions.
Continue to monitor and obtain the Behavior Management & Psychotropic Medications consents. Prepare the documents for the SCC-Human Rights Committee meetings. Ensure that the minutes for the SCC meetings are accurate and up to date along with appropriate documentation.
Monitor and oversee the completion and accuracy of consumer records as per NYSOPWDD and agency guidelines for ICF and Day Habilitation program.
Advocate for consumer services, i.e. case management, group facilitation, planning and provisions for recreation/respite, budgeting, etc…. while ensuring protective oversight.
Participate in the ITT process. In the absence of SW, you will carry the case load until there is a replacement staff due to your historical experience/background in these areas.
Provide, monitor and oversee provision of clinical services (Nursing, occupational therapy, physical therapy, nutrition, speech, psychological/psycho-social services) to identified individuals and ensure that the maintenance of case records/notes/treatment plans/progress notes, menus for the individual service plans/clinical reviews and all other pertinent clinical information is up to date and in compliance in the IRA’s, ICF, and Day Habilitation Program.
Work in conjunction with Nursing Supervisor and Nursing Staff to ensure necessary department meetings are conducted, maintenance of case records/notes, and nursing staff duties in the department are completed in a timely fashion.
Work in conjunction with Social Worker to invite and obtain necessary clinical documents for ISP and ITT reviews.
Write and conduct psycho-socials as well as social worker at the facility. (Shared Responsibility)
Facilitate group meetings such as the Circles Group
Accompany consumers to and from activities and appointments, i.e. social, recreational, medical, dental and other appointments when necessary.
Attendance/participation in citywide committees, representing CAHS, Inc. in a professional, prompt and punctual manner.
Act as a liaison between CAHS, Inc. and other community-based organizations/services/Care Coordination in the event of a critical and/or emergency incident.
Notify AED or agency designee in the event of a critical and/or emergency incident.
Carry out any additional responsibilities as defined by the Chief Executive Officer, Assistant Executive Director, Agency Designee and consumers.
Appropriate dress for this position is in accordance with the agency’s dress code.


SUPERVISION
Plans, and trains student interns and volunteers on an as need basis in accordance with regulatory and accreditation rules, regulations and standards as they apply to programs/services for persons with developmental disabilities. Collaborates with program management personnel to ensure that services reflect current regulations and best practices. Provides individualized staff training/remediation as needed.
Participates in Quality Assurance/QI activities and pre-audit preparations, including staff and consumer observations and folder reviews to ensure quality of service and compliance in such areas as individual rights, abuse, behavior programming, fire safety, etc.
Reviews and monitors staff performance and provides verbal or written corrective feedback, including disciplinary actions when indicated, i.e. unannounced site visits (with Residential Coordinator Involvement)
Responsible to conduct regular team meetings with appropriate staff to apprise them of all pertinent and current information, which may directly or indirectly affect the consumers’ families and program.
Meet with clinicians (physical therapist, occupational therapist, nutritionist, etc.) for monthly meeting to review and address any issues/concerns/new information pertaining to the consumer(s). Invite SW or MSC or identified staff to participate in monthly clinical meeting.

Educational Requirements:

Masters Degree in Social Work, Rehabilitation Counseling or related Human Services field or Bachelors degree and three (3) years of experience in OPWDD field.
LCSW preferred
Doctorate in Psychology or Social Work preferred


Skills and/or Experience Required:

Strong organizational planning and management skills.
Two or more years’ experience in supervising at least one of which must include administrative duties. Additionally, must have experience working with the developmentally disabled or similar population.
Ability and willingness to master computer skills.
Demonstrated competency in written, verbal and computational skills to present and document records in accordance with program standards.
