Essential Functions Statement(s)
� Assists assigned team, nurse or social worker with the coordination of health care activities for members with duties
including but not limited to:
� Conducting telephone interviews with member, significant others and family members to determine if care needs are being
met or if additional services are needed
� Ensuring members make and keep scheduled medical appointments
� Assessing member's ability to follow up and implement care plan activities
� Identifying gaps in care for members and arranging appropriate and timely resolution with internal and external resources
and agencies
� Scheduling assessments; coordinating and obtaining medical records; ensuring processes are completed within state
required timelines
� Utilizing the case tracking system to complete tasks, update care plans, assessments and interventions
� Performing member surveys, accurately documenting and reporting results
� Identifying need for and facilitating member transportation
� Prioritizing members according to intensity, need, adherence and required follow-up
� Promoting member self-management and empowering members/caregivers to achieve maximum levels of wellness and
independence
� Making appropriate referrals to other programs to address the member's needs
� Working in conjunction with others on the team to promote collaborative teamwork
� Participating in interdisciplinary team conferences as needed
� Interacting with internal and external customers in a manner, which is professional, respectful, positive, helpful and
promotes trust
� Internal data collection as required for program needs
� Maintaining professional growth and development
� Complying with all requirements of the Care Management Model of Care
� Identifying the need for development or change in department processes to improve department efficiency/efficacy and
offers specific suggestions for improvement
� Contributing to Care Management Policies and Job Aids as directed
� Participating in department audit activities
� Other duties as determined by role
