Certified Pharmacy Technician/Patient Care Coordinator

A Certified Pharmacy Technician/Patient Care Coordinator job in Spartanburg, SC is available courtesy of Adecco Medical & Science. You will be responsible for conducting in-home or telephone outreach to identify and document patient's current medication regimen. You will work closely with patients, consult with Pharmacists, and Physicians to reduce patient risk of readmission to hospital, emergency departments, or other acute care facilities. Responsible for informing patients of the variety of pharmacy services offered including specialty packaging and home-delivery services. This role is a highly patient focused position. This is a temporary role starting in February 2019 and will be a minimum 3-6 month+ contract. The work schedule will be Monday-Friday from 8:00am-5:00pm (34-40 hours/week). You MUST have reliable transportation since you will be making in-home visits.

The Certified Pharmacy Technician/Patient Care Coordinator job responsibilities include:

Essential Duties and Responsibilities

Conduct in-home or telephone visits based on program guidelines.
Develop rapport with patients and/or caregivers to gain confidence and trust.
Complete and document patient health questionnaires.
Document and communicate medication concerns to consultant pharmacists, physicians, or other healthcare providers.
Complete inventory and reconciliation of current medications.
Maintain communication with patients throughout the duration of the program.
Obtain insurance information.
Create a patient profile from a scanned or faxed image by entering correct patient and provider information to ensure all other groups can successful serve the patient.
Communicate with providers regarding referral status, prescription clarification, missing information or patient concerns so that all information is collected and accurate before it reaches other teams for furthering processing.

Qualifications

Minimum High School diploma
Registered technician through the South Carolina Board of Pharmacy or achieve registration within 1 month of employment
Requires 2+ years of Pharmacy Operations experience.
Current CPhT (Certified Pharmacy Technician) certification required
Basic computer skills.
