SUMMARY:
Person conducts high-touch phone outreach to engage Medicaid enrollees who are eligible for Health Home benefits. Engages, educates and disseminates information about the program.
Client population are individuals with chronic conditions.

Specific Duties:
1. Outreach phone calls to engage clients
2. Occasional liaison work between client and provider agency
3. Data entry
4. Various other clerical duties


Must have qualifications/experience:
1. Compassion for individuals experiencing chronic health conditions
2. Great communication skills
3. Able to work independently
4. Able to connect and build relationships with clients
5. Basic computer skills (familiarity with MS products and internet).


Shift Start/End Time: Monday to Friday - flexible 20 hours/week - 11 AM to 5 PM




This is a 6+ month contract opportunity with possibilities for extension and full-time conversion.
