A LPN or RN Case Manager in Tampa, FL is now available courtesy of Adecco Medical & Science. In this role you will be making outbound calls to schedule patients for health screening events throughout the FL market.



Document call outcome on Social Care and Microsoft excel.
Maintain a minimum daily call count of 80 calls per day and 30% conversion rate.
Assists callers with general contacts including, but not limited to, appointment scheduling, general facility and provider information, general requests, medical records requests and issue resolution.
Verify and update patient information in the clinical information system.
Contact patients to remind of appointments.
Review patient chart (in EMR) to ensure core measures are being addressed and met per protocol and takes appropriate action when they are not, i.e., schedule services.
Contact patients to communicate special outbound call campaign messaging.




Qualifications:


Must have a LPN or RN license in good standing in FL
Must have at least 3 years of experience as a LPN or RN
Must have case management experience in utilization, and disengaged membership



Hours are Monday through Friday 8am-5pm
