LPN's or RN's - Case Management/Utilization Review - Tampa, FL

**** Call Center Position which requires an active LPN license or RN License, that is reliable and has Case Management experience in Utilization, and disengaged membership. With at least 3+ years as an active LPN or Registered Nurse in an acute care facility or hospital is preferred. ****

- LPN or RN
- 3 years of current experience.
- Case Management experience, within Utilization.
- Acute/Hospital experience.
- Proficient in excel is a must.
- Experience with geriatric population.
- Problem solving skills.
- Ability to work independently.
- Any prior call center experience.

Job Responsibilities:
- Make outbound calls to schedule patients for health screening events throughout our Florida and Texas Markets.
- Document call outcome on Social Care and Microsoft excel, or the member's electronic Medical chart.
- Maintain a minimum daily call count of 80 calls per day and 30% conversion rate.
- Assists callers with general contacts including, but not limited to, appointment scheduling, general facility and provider information, general requests, medical records requests and issue resolution.
- Verify and update patient information in the clinical information system.
- Contact patients to remind of appointments.
- Review patient chart (in EMR) to ensure core measures are being addressed and met per protocol and takes appropriate action when they are not, i.e., schedule services.
- Contact patients to communicate special outbound call campaign messaging.

Shift: Monday-Friday 8AM-5PM, but unsure of training schedule at this time.

Contract Length: December 2, 2019 to March 6, 2020.
