Patient Care Coordinator

Specific Duties:
• Answer incoming calls, assisting members and other health care providers with pharmacy benefit questions and escalating complicated drug related calls to the clinical pharmacist.
• Assist the clinical pharmacist in implementing member and provider outreach programs related to HEDIS/P4P, CMS Star Ratings, and generic utilization.
• Access, input, and retrieve pharmacy data, and ability to run pharmacy reports.
• Assist pharmacy team in identifying high volume/high cost pharmacy utilizing members from data reports.
• Maintain customer/patient confidentiality according to HIPAA and company standards.
• Perform administrative/clerical related duties as required by the clinical pharmacy team.
• Responsible for inputting and processing phone and faxed pharmacy projects.
• Review monthly quality data files identifying members out of compliance for CMS Star Measures.
• Develop education materials to members and physicians assigned by the pharmacist.
• Perform members outreach to monitor and assess medication compliance.
• Prepare reports by collecting and summarizing information.

Must have qualifications/experience:
• Must be 18 years or older and have a high school diploma or GED completed.
• Proficient in the utilization of software programs such as Microsoft Office, which includes the ability to learn new and complex computer system applications.
• Typing ability 30+ WPM preferred.
• Ability to document chart notes, and effectively present information and respond to questions from those within the organization as well as to groups outside of the company.
• Effective verbal and written communication skills.
• Able to organize and manage multiple priorities.

Shift Start/End Time: 8:30 am-5:00pm
