Pharmacist- San Antonio

Pay Rate: 50/hr


Specific Duties:
1. Specific Duties:
1. Conduct Comprehensive Medication review for select Medicare beneficiaries
2. Utilize prescription and medical claim data to generate clinical recommendations according to Medication Therapy Management and Retrospective Drug Utilization Review program protocols
3. Follow all policies and procedures related to job
4. Performs all other related duties as assigned to meet departmental objectives.
5. Knowledgeable of current and emerging clinical trends
6. Provide clinical recommendations pertaining to, but not limited to, gaps in care, high risk medications, compliance and adherence, drug interactions, therapeutic substitution, and generic substitution
7. Utilize health plan’s formulary information to guide appropriate medication recommendations

8. Assist with all pharmacy-related CMS quality measures

Must have qualifications/experience:
§ Doctor of Pharmacy degree
§ 2 or more years’ experience as a clinical pharmacist
§ Excellent written and verbal communication skills
§ Excellent relationship building skills
§ Planning and organizational skills
§ Strong interpersonal skills. Ability to work independently, as well as a member of a team
§ Strong analytical and PC operation skills
§ Experience working with provider relations, working within various payor environments, and risk management

Preferred Education, Experience & Skills
§ Fluent in Spanish Language a plus
§ Certified Geriatric Pharmacist
§ Certification in Medication Therapy Management (MTM)
§ Experience with Medicare guidelines and measures

Shift Start/End Time: 9am to 6pm
