PAY RATE: 33/HR

Position Summary:
The Case Manager will be responsible for ensuring SYHC’s female patients (24-64 years old) receive their Pap tests and provides appropriate follow-up when necessary.

Essential Functions of the Job:

Identify SYHC patients who need cervical cancer screening services: 1) women who have never received Pap tests, 2) women who have not received Pap test in the past 2 years, and 3) women who will be due for Pap tests
Schedule patients for a Pap test and send reminder cards of their appointments.
Serve as liaison between the patient and SYHC’s clinical staff.
Provides Case Management duties for patients to ensure adherence to SYHC protocols and standards.
Provide cervical cancer education as requested.
Work with clinic’s case managers to appropriately schedule and/or reschedule patients’ Pap tests and follow-up/consult appointments according to providers’ recommendations.
Track patients through all stages of cervical cancer screening process in i2iTracks, including reminding patients of upcoming Pap test appointments, reminding patients that they are due for a Pap test, informing patient of Pap test results, scheduling follow-up appointments if necessary, and rescheduling appointment if patient missed their appointment.
Work with PCMH Implementation Team to identify women in need of, but are not receiving, cervical screening services.
Once identified, contact women who are due for, but not receiving cervical cancer screening services.
Increase the health literacy of patients regarding cervical cancer and the importance of receiving Pap tests regularly and timely.

Additional Duties and Responsibilities:

Maintains and complies with established departmental policies and procedures, objectives, quality assurance program, safety environmental and infection control standards.
Enhances professional growth and development through participation in educational programs current literature, in-service meetings and workshops.
Attends meetings as required.
Maintains program specific records as required; submit reports, including activity, contact, and mileage reports in accordance with agency policies and procedures.
Maintains case management and productivity log.
Maintains client contacts and case records. Data to be tabulated monthly.
Attends training programs as authorized or directed.
Performs other related duties as assigned or requested.

Job Requirements:

Education Required (Minimum level of education):
High School diploma or GED equivalent.
Completion of a Medical Assistant Certificate.
Preferred: Completion of Licensed Vocational Nursing Program.

Certifications/Licenses Required:
Medical Assistant Certificate.
CPR certification.
Valid California driver’s license.
Preferred: Licensed Vocational Nursing program.

Experience Required (Minimum level of experience):
Externship required by Medical Assisting Program.
Preferred: Completion of clinical rounds required by LVN program.

Verbal and Written Skills Required to Perform the Job:
Bilingual (Spanish-English).
Excellent fluent written and verbal communication skills.

Technical Knowledge and Skills Required to Perform the Job:
Medical terminology and ICD-9/CPT coding, typing skills (at least 30 wpm).
