The Scheduling Coordinator maintains all clinical field staff schedules by coordinating appropriate visits to include: admissions, discharges, resumption of cares, recertification visits, and routine follow up visits on a daily basis. This position assist the Clinical Management Team in creating, collecting and entering the case manager schedules into a master document that is kept current at all times.

Qualifications: High School Diploma or Equivalent.


1 to 2 years of experience in a healthcare setting preferred.

Must have professional and customer-service-driven phone and communication skills.

Ability to communicate effectively and tactfully with management, clinical staff and office staff in a constructive, goal directed, and professional manner.

Ability to communicate effectively and tactfully with patients, doctors, and all customers in a professional manner.

Strong ability to organize and prioritize workload on a regular basis based on the needs of customers patients and the clinical department.

Ability to be flexible and to follow verbal and written instruction in a fast-paced team-oriented environment.

Proficient in completing routine paperwork, multi-tasking, and providing appropriate follow-up as needed.

Ability to present reports to small and large groups upon request.

Strong knowledge of Microsoft Office Systems (i.e. Word, Excel, PowerPoint, etc.) and computerized systems in general required

Must be able to type at least 35 wpm

Ability to develop and maintain filing systems.

Participates in maintenance of quality, safety, and/or infection control standards.

Working knowledge of Medical terminology.

Must maintain confidentiality of records and information at all times. Responsibilities:

Manages all aspects of the clinical field staff schedules on a daily/weekly basis while ensuring admitted patients are seen as ordered by the referring physicians.

Compares, monitors, and makes any necessary changes to the actual/planned weekly visit schedule in Homecare Homebase (HCHB) scheduling console

Regularly assists the clinical field staff in meeting productivity expectations by adjusting the schedule as needed.

On a weekly basis, reviews Admitted Patient Census Report to ensure all patients are scheduled with clinicians as ordered by the physician.

On an on-going basis, reviews the Case Manager and Patient Assignment Report to ensure all new patients are assigned to a case manager and are scheduled accordingly.

Makes copies of new patient information to include the history/physical, orders, hospital reports, labs, clinical notes, and any other necessary patient documents and disseminate copies to the assigned clinicians mailbox. Notifies the Case Manager of the new assignments and provides staffing coverage until the Case Manager is available to visit their assigned patients.

Assembles all required documents needed for Recertification visits and ensure those visits are scheduled at least 5 days prior to the end of the episode.

Schedules all disciplined staff evaluations with the appropriate field staff within 48 hours.

Reports to the Clinical Management Team whenever it is not possible to get visits covered after all other means have been exhausted.

Assembles all disciplined staff schedules prior to the upcoming work week and makes needed changes as necessary.

Verifies Per-Diem Staff availability for the upcoming work week.

Ensures compliance with all State and Federal regulations/ practices.

Attend and participate in all agency wide meetings/ in-services related to the intake position as appropriate.
